FMLA

To submit an FMLA request please upload your documentation from your employer. Please have on hand your surgery date, your return to work date, and your occupation/ type of work.

Timeline:
As a general guideline KC Bariatric gives 2-3 weeks for a sit down desk/ job and 4-6 weeks for a more physical job ( a job requiring 15 lbs of lifting or more).
