FMLA

To submit an FMLA request please send your documentation from your employer to your Case Manager. Please have on hand your surgery date, your return to work date, and your occupation/ type of work.

You can also fax your documents to 913-677-1540.

If your Case Manager is not listed below, please send your documents to staff@kcbariatric.com.

Timeline:
As a general guideline KC Bariatric gives 2-3 weeks for a sit down desk/ job and 4-6 weeks for a more physical job ( a job requiring 15 lbs of lifting or more).
